Please bring your current insurance information to every appointment. We copy your insurance information each time you come to our office. We will also ask you to update your address, phone numbers, and email address at each appointment. These policies help us maintain contact, and are extremely beneficial and important to ensure your appointments and procedures are paid for by your insurance company.
We accept most insurance. You should be able to check with your insurance company to see if Bradley J. Burket is contracted with that company.Quality patient care is our top priority! We strive to provide medical and dental care to everyone and attempt to work out payment plans with our patients if necessary.
However, we do maintain certain policies: Co-pays are due at each visit. If you do not have a co-pay, then you may be asked to pay your portion of your bill that is not covered by your insurance.
We require pre-payment for some procedures including, but not limited to: pre-natal care (payments throughout pregnancy,) vasectomies, IUD placement, and dental procedures that require use of a lab such as crowns, veneers, and dentures.
Pre-authorization: For cases requiring pre-authorization, our billing manager will assist you, if applicable, to get your procedures covered.
