WELL CHILD EXAMS are recommended at 2 days old and 2 weeks old, but no vaccines are given at that time.

WELL CHILD EXAMS with immunizations are recommended for the following ages:
    Well Child Exams 2 months
    Well Child Exams 4 months
    Well Child Exams 6 months
Well Child Exams Between the ages of 4 and 5
Well Child Exams If a patient is missing immunizations or is behind the
immunization schedule, vaccines will also be given at
      9 months of age.

Toddlers ages 2 and 3 should have a well child examination but no immunizations are given at that time.

After age 5, Well Child Exams are completed every 2 years.

Visit the Crook County Health Department website for a useful immunization chart.  You can download or print this form to keep your own child's records.  It's helpful to have this information on hand at home once your child reaches school age.
